Who can make a hospice referral? Hospice certification must be made by a physician, but requests for information may be made by physicians, patients, family members, friends, clergy and other members of the community.

Introductory visits
Community Hospice offers introductory visits at no cost. Introductory visits can provide patients and families with an opportunity to discuss the benefits of early hospice intervention and what that care will involve. Our team of professionals will meet with the patient and their family to offer information and assistance while listening to concerns and helping to alleviate fears and anxiety.
For referrals or more information, contact Community Hospice at (251) 943-5015.
